Before participating in any service, it is REQUIRED that you complete a general Consent & Liability Form.

In addition to the required Informed Consent & Liability form, there are also session-specific intake forms which cover information pertinent to your preferred modality. If you are unclear which form to complete, or experience any issues, please reach out at info@tranquility-summit.com. Completing these ahead of time allows us to provide you with the highest level of service.

Energy Healing
If you have selected a service that offers only Energy Healing, this is required to be completed prior to your session start.

Complimentary Intake
If you have only scheduled a complementary intake call, there are no required additional forms to complete prior to our discussion. That is intended to be a forum for you to ask me whatever questions you may have.
