I understand that  neither reiki practitioners or hypnotherapists diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. 

I understand that these sessions are not meant to replace treatment by established medical practices & professionals, but to complement them. These sessions can also create a more synergetic result with other interventions, treatments & modalities.

I understand that it is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have or may experience later on. If experiencing thoughts of self harm, emotional and or mental distress, and or personal emergency, I must call 911 or local emergency services immediately.

I understand that there are no guarantees as to the results of treatment, mentoring & or education are expressed or implied by the practitioner.

I agree to raise any questions or concerns about anything I do not understand prior to our session(s).

I agree to fulfill all payments in full & on time.

I understand all sessions must be paid in full prior to the appointment—no exceptions. Appointments are confirmed once payment is received.

I understand that all payments are final.

I agree to increase my water intake prior to & after any energy healing work I may participate in during our session(s)

I agree to consider any suggestions that the practitioner may raise concerning referrals to other health care practitioners, homework, or my desired focus/introspection.

 I agree to Take full responsibility for my own health care, actions & effort.

I am of legal age and in consideration of my acceptance as a participant in hypnosis, energy healing, and coaching sessions, training, seminar or any other Tranquility Summit, Inc. production, I for myself, my heirs, executors, administrators and assignees, do hereby release and discharge Tranquility Summit, Inc.  and Ashleigh and any of their employees or other participants from all claims of damages, copyright, demands or actions whatsoever in any manner arising from my participation. Further,

I understand that it is advised to drink plenty of water prior & after my energy healing classes & or session(s). By failing to increase my water intake, I am increasing my chances of experiencing potential side effects.

I understand the Potential side effects of reiki & energy healing include but are not limited to the following:

Dizziness , Headaches, Vertigo, Nausea, Fatigue, Heightened Emotionality, Tingling Body Sensations & Changes in Digestion. 

I understand my refer me to a licensed health care provider if this work extends beyond her current scope-of-practice.

I voluntarily assume full responsibility for any decisions and actions arising from the coaching process and acknowledge that such decisions and actions are my own.
